We will accept your insurance after we have verified that you have satisfied your annual deductible and if the reason for your visit to the doctor is a covered medical condition under your policy.
Co-payments and deductibles are due in advance at the time of your visit. If the nature of the services provided is not covered under your insurance policy, you will be responsible for such payment. |
